Works under the supervision of the Chief Nursing Officer, Lifelong Health. Responsible for implementing and evaluating evidence based policies and procedures for resident care areas in skilled nursing and assisted living facilities in Lifelong Health Division. Evaluates quality outcomes, collaborates with facility leadership and Director of Quality to implement action plans/interventions to facilitate quality improvement. Collaborates with Director of Quality to ensure effective management and follow through of QA processes for clinical services. Provides training and education, mock and survey support, quality monitoring, investigative and technical support for assigned facilities. Assists nursing leadership with plan of corrections and action plans as needed to ensure compliance with all state, federal and regulatory requirements. Develops training for nursing leadership to support nursing leadership on-boarding. What you will do Develops nursing leadership through on-going training and providing orientation to new nurse leaders. Includes DON, ADON, Nurse Manager and Quality Assurance. May need to temporarily cover vacant DON positions. Implements and evaluates effectiveness of evidence based policies and procedures for Resident Care areas in Skilled and Assisted Living Facilities through routine visits to facilities rounding, observation, education, and discussion with facility staff, including physician and advanced practice providers. Identify and implement clinical best practices resulting in excellent patient outcomes. Assists facility in the development of a Plan of Correction (POC) and/or action plans to address deficiencies found during mock surveys, focused audits and federal, state and local surveys. Monitors compliance with POC and action plans to assure correction of identified issues and reports compliance. Participates in facility QAPI meetings. Monitors facility quality indicators and scorecard/dashboard results regularly to identify improvement opportunities and determine focus for site visits. Assists facilities in the development of systems & processes to correct deficient areas noted through quality improvement activities and survey processes. Reviews quality measures and quality data (CMS, 5 Star, CASPER, PointRight, etc.) to assist facility leadership in development of Performance Improvement Plans (PIP). Assists facility staff in assuring and maintaining compliance with federal, state and local regulations related to resident care services. Performs focused audits based on departmental goals and needs. Performs mock surveys and follow-up to mock surveys in each facility based on departmental goals. Responsible for conducting mock surveys, follow ups, and serves as a resource during surveys. Reviews new regulatory directives and proposes changes based on industry best practices. Provides support to facility staff before, during and after regulatory surveys. Provides technical assistance to direct care staff as needed to improve efficiency and effectiveness of resident care. Provides support and guidance to nursing leadership in the performance of daily operations. Provides inservice training and orientation to facility staff regarding survey process, risk management, corporate compliance, performance improvement, and clinical care. Facilitate outcomes management initiatives to support quality outcomes monitoring scorecard/dashboard results regularly to identify areas for improvement. Assists facilities in the development of systems & processes to correct deficient areas. Reviews quality measures and quality data (CMS, 5 Star, CASPER, PointRight, etc.) to assist facility leadership in development of Performance Improvement Plans (PIP). Assists with assuring evidence based practice is incorporated into LLH policies and procedures. Assists Facilities in assuring compliance with regulations relating to state and federal requirements and guides the facility in investigating Facility Reported Incidents with appropriate investigation, documentation and action plans.
