Responsible for individualized client services provided in a setting providing addictions services. Duties include assessing, planning, implementing, monitoring, and evaluating client behaviors and outcomes while ensuring appropriate level/type of service is provided. Position Description: With close supervision; conducts interviews and administers assessments and screening instruments as defined by certification Maintain caseload and productivity by ensuring regular contact with clients to monitor and assess progress Lead educational groups and/or facilitate group curriculum Write service plans, review progress and determine updates needed in collaboration with client and team Utilize approved best practice model working with clients in individual or group setting Deliver on-going services to clients which may include: crisis intervention, advocacy, medication monitoring, and referral and linkage to community resources as needed Ensure compliance with applicable standards and agency policies Document all clinical activities to meet specific program, funder and regulatory criteria timely and with accuracy Participate in team meetings and assist in making recommendations for client status changes Adhere to professional standards as outlined by protocols, rules and regulations Other duties as assigned
