About The Position

We are seeking a skilled Registered Nurse (RN) to join our team and provide clinical support to Community Health Navigators (CHNs) in managing high-risk members through the Enhanced Care Management (ECM) program. In this role, you will provide guidance, training, and clinical expertise to CHNs, ensuring that they have the tools, knowledge, and resources to successfully support members with complex clinical needs as well as health-related social needs (HRSN). This role does not involve direct patient care but focuses on empowering the CHNs to deliver effective care and achieve positive member outcomes.

Requirements

  • Active Registered Nurse (RN) license in the state of California, or ability to gain within 10 weeks of employment
  • Minimum of 2 years of experience in a clinical or community health setting
  • Experience in managing chronic health conditions, care coordination, and patient education is highly desirable
  • Can write user stories that developers do not have to constantly ask questions about
  • Strong clinical knowledge of common chronic conditions (e.g., diabetes, COPD, hypertension, heart failure, etc.)
  • Ability to maintain clear professional boundaries while providing support and guidance to CHNs
  • Excellent organizational skills with the ability to manage schedules and provide efficient support to multiple CHNs
  • Strong problem-solving and critical thinking abilities, particularly in complex care situations
  • Capacity to work independently and as part of a team in a virtual operating environment
  • Compassionate, empathetic approach to supporting CHNs and enabling their success in managing high-risk members
  • Cultural competency, with the ability to work with a diverse group of CHNs and community members
  • Strong interpersonal and communication skills, both written and verbal

Nice To Haves

  • Bachelor of Science in Nursing (BSN) nursing preferred, Master of Science in Nursing (MSN) a plus
  • Experience in training and education is highly desired
  • Pediatrics experience a plus
  • Behavioral health experience a plus
  • Knowledge of CALAIM, ECM, CS programs and HEDIS measures is a plus
  • Familiarity with community-based health resources and services
  • Experience with pediatrics and/or behavioral health issues is highly beneficial
  • Proficiency in using technology including EHRs, health plan portals, and Salesforce is a plus
  • Multi-lingual capabilities are a plus but not required

Responsibilities

  • Conduct clinical rounds where Community Health Navigators (CHNs) present Care Plans for high-risk members. Provide clinical feedback and guidance to ensure that both medical and social aspects of care are properly addressed.
  • Lead training sessions for CHNs on various health-related topics, including diabetes management, COPD, hypertension, medication adherence, and other chronic conditions, as well as social issues like food insecurity, housing support, and access to healthcare services.
  • Offer clinical guidance and support to CHNs when they encounter challenging or escalating situations with high-risk members, helping with complex care decisions or clinical interventions.
  • Assist CHNs in setting and implementing SMART goals for members, including helping to define achievable interventions and tracking progress.
  • Provide advice to CHNs on overcoming barriers when implementing care plans and SMART goals, especially in difficult clinical situations.
  • Offer guidance to CHNs on how to address mental health or substance abuse concerns when escalations occur, helping them coordinate appropriate resources or interventions.
  • Review and evaluate patient treatment plans, offering advice and insights to CHNs on updates or modifications as needed to ensure optimal care delivery.
  • Assist with onboarding new CHNs and provide ongoing education and support to ensure they are equipped to manage members with chronic health conditions and complex care needs.
  • Deliver targeted, one-off trainings to refresh CHNs on specific topics, such as navigating housing resources, understanding Medi-Cal benefits, and engaging members with Serious Mental Illness (SMI) or substance use disorder (SUD).
