PRIMARY FUNCTION/GENERAL PURPOSE OF POSITION: Delivers patient care utilizing the nursing process through data collection and assessment, identifies and determines the priority of patient’s problems/needs, and evaluates the process and outcome of nursing care; coordinates, provides and directs patient teaching activities and coordinates the care provided by health team members. ESSENTIAL JOB FUNCTIONS: In collaboration with the interdisciplinary care team, conducts initial and ongoing patient assessment, analyzes assessment data, creates a plan of care, implements treatment and evaluates treatment effectiveness; administers medications in a safe manner consistent with the State of Practice and Bon Secours Mercy Health policies and procedures; serves as point of contact for patients' care coordination throughout hospital departments. Acts as a patient safety advocate by participating in ongoing quality improvement in the department. Incorporates the professional practice model into care delivery Provides focused assessment to evaluate patients and their support systems for real and potential health and safety issues, educational needs, and resource availability upon initial interaction, throughout care and/or treatment, and at times of transition in collaboration with the multidisciplinary team. Utilizes multiple means of communications including telephone triage, in-person interaction and the use of current and future technologies. Facilitates patient access and progression through the Advanced Heart Failure Center. Provide education to staff, patients and families as needs are identified. Provides clinical and administrative program support to the cardiologist (s) and Advanced Practice Clinicians [APC (s)J associated with the Advanced Heart Failure Clinic, industry sponsors as well as staff and patients. Maintains communication with referring clinicians

Requirements

  • BLS Basic Life Support - American Heart Association
  • Associate of Nursing, nursing
  • 3-5 years cardiac surgery and/or cardiology step down or critical care experience as a Registered Nurse
  • Experience within a physician practice setting.
  • Working knowledge of medical record format and content of patient records.
  • Ability to review records, read physician documentation and verify patient identification.
  • Good interpersonal skills and ability to communicate with practice managers and other medical group staff/leadership.
  • Work requires a high level of accuracy with the ability to concentrate on detailed information for extended periods of time.
