About the position

The Clinical Rehabilitation Specialist acts as a coordinator between internal and external referral sources and acute inpatient rehab operations. This position is responsible for establishing relationships with case managers, physicians, payors, and other referral sources to ensure appropriate patient placement through clinical screenings and assessments in collaboration with both hospital Case Management and the Rehab Medical Director. The Clinical Rehabilitation Specialist works closely with hospital case management to ensure a responsive and smooth flowing referral and admission process for the inpatient acute rehab program. They will review patients daily until they have strong clinical evidence to request a physician order for a formal pre-admission assessment for acute inpatient rehab. Once Case Management has completed the Patient Choice process, the Clinical Rehabilitation Specialist will assist and coordinate the intake and pre-admission screening process, accurately documenting evidence of the patient's compliance with the established rehabilitation admission criteria. They will also perform on-site and off-site reviews of patients to assist Case Management in identifying patients that could benefit from rehab services and provide information related to inpatient rehab to patients and family members.

Responsibilities

  • Ensure appropriate patient placement through clinical screenings and assessments.
  • Work closely with hospital case management to ensure a smooth referral and admission process.
  • Review patients daily for clinical evidence to request a physician order for pre-admission assessment.
  • Assist and coordinate the intake and pre-admission screening process.
  • Accurately document patient compliance with rehabilitation admission criteria.
  • Perform on-site and off-site reviews of patients to identify those who could benefit from rehab services.
  • Provide information related to inpatient rehab to patients and family members.
  • Communicate expectations and requirements for participation upon admission.
  • Establish and maintain effective working relationships with all referral sources.
  • Participate in market analysis and development of business plans for the Rehab Unit.
  • Manage time proficiently and prioritize commitments to referral sources.
  • Develop a backup system to handle calls when unavailable.

Requirements

  • Must have an active license in one of the following: Physical Therapist, Speech Therapist, RRT/Respiratory Care Practitioner, State Registered Respiratory Therapist, Occupational Therapy, or Registered Nurse.
  • 2 years of clinical experience in an inpatient setting required.
  • Strong presence within hospitals is required.
  • Good communication and relationship building with families and hospital staff.
  • Experience as a clinical liaison coordinating admissions for inpatient rehabilitation preferred.
  • Clinical experience in the care and management of the Inpatient Rehabilitation patient preferred.
