About The Position

As a Clinical Rehab Specialist with Parham Doctors Hospital, you will act as an inpatient rehabilitation expert, establishing relationships with case managers, physicians, payors, and other referral sources. You will ensure appropriate patient placement through clinical assessments and screening in collaboration with the Rehab Medical Director. This position is focused on moving appropriate candidates into inpatient rehabilitation at the right time.

Requirements

  • Basic Cardiac Life Support must be obtained within 30 days of employment start date.
  • Associate Degree.
  • Registered Nurse (RN), Occupational Therapist (OT), Licensed Physical Therapist (LPT), or Speech-Language Pathologist (SLP).

Responsibilities

  • Assist and coordinate the in-take and pre-admission screening process.
  • Accurately document evidence of rehab admission criteria on Pre-Screen evaluation in a timely and legible manner.
  • Complete pre-screen updates within 48 hours of admission.
  • Communicate admission decisions to referral sources within 2 hours from time of referral receipt.
  • Perform on-site and off-site clinical evaluations to assist the Rehab Medical Director in determining patient admission appropriateness.
  • Provide information related to inpatient rehab and program features to patients and family members.
  • Communicate expectations and requirements for participation upon admission to the rehab program.
  • Offer tours of the rehab unit to patients and families, coordinating if unable to provide personally.
  • Secure information relating to patients' resources and healthcare insurance benefits.
