About The Position

Function as a liaison between internal/external referral sources and inpatient rehabilitation
operations.
Establish relationships and aid with communication to case managers, physicians, payors, patients, families and referral sources in order to generate referrals and drive patient admission volume in the inpatient rehab.
Ensure appropriate patient selection through clinical chart assessments during on/off-site clinical
evaluation of patients in collaboration with the physician.
Complete in-service and educational opportunities for physicians, case managers, interdisciplinary members and other professionals. Serves as an educational resource to patients, family members and caregivers.
Participate in the development and implementation of business plans for the rehab unit and contributes to the development and maintenance of referral source promotional information.

Requirements

• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
• Rehab Therapist specializing in Occupational Therapy (OTR) credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.
• Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.
• Speech-Language Pathologist credentialed from the Wisconsin Hearing and Speech Examining Board obtained prior to hire date or job transfer date.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.
• Drivers License preferred.
• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

• Previous experience in long term care/inpatient rehab or acute rehab setting.
• Ability to be self directed, prioritize patients to be seen, comfortable with electronic health record navigation.
• Excellent verbal and written communication.
• Ability to assess the patient and communicate to a physician if the patient is appropriate for an inpatient rehab setting.
