About The Position
The Clinical Rehab Navigator at Ascension Providence serves as a vital link between internal and external referral sources and inpatient rehabilitation operations. This role focuses on enhancing communication and relationships with case managers, physicians, payors, patients, families, and referral sources to drive patient admissions and ensure appropriate patient selection through clinical assessments. The navigator also plays a key role in educational initiatives and the development of business plans for the rehab unit.
