Professional Qualifications: Clinical Referral Specialist will have a degree or experience in Nursing, Social Services, Marketing, Business or a related field. Verbal communication skills, excellent customer service skills, and strong organizational skills are important components of this position. Intake Coordinators are continually learning and growing by participating in educational sessions, in-service training, and demonstrations. Some training may require out-of-town travel. Key Result Areas: · Consistently uses the Vetter Health Services Mission, Vision and Values to guide everyday decisions and actions. · Demonstrate a consistently positive, respectful and constructive attitude with both clients and staff. · Demonstrate initiative, active problem solving and a continual desire to improve customer service. · Serve as an advocate for each client to ensure their physical, psychosocial, spiritual and restorative needs are being met. · Create and maintain an environment that allows each client to adjust to their limitations and recognize the dignity and worth of each individual. · Participate in in-service educational classes and meetings directed at improving your ability to meet the needs of the clients. · Maintain and follow established safety precautions and infection control procedures when performing day-to-day responsibilities. · Know, understand and perform responsibilities that meet regulatory and company standards. · Serve as an effective teacher, model, and consultant for other staff. · Be a reliable and positive member of the agency's team. · Create a cooperative working relationship with other departments. · Serves as an active member of the agency census building team. Represents the company with professionalism to the medical/hospital community referral sources (ie. care coordinators, discharge planners, social workers & physicians). Develops and maintains positive relationships to gain referrals May communicate with potential patients and families to educate them of the services agency offers. Coordinates pre-authorization for managed care admissions. Current understanding of all Medicare Regulations/Requirements · Coordinates and communicates patient referrals and processes information. · Tracks pre-admission referral activity daily/weekly

Requirements
