About The Position
SRMH Cardio Practice in Harrisonburg VA is looking to hire a Clinical Referral Coordinator, Licensed Practical Nurse. The Clinical Referral Coordinator, Licensed Practical Nurse evaluates and processes referrals by receiving and responding to incoming calls from referral sources or potential patients. Record the outcome of calls and make follow up calls as necessary. Enter referral information in system including clinical information and orders for services. Develop and maintain a working knowledge of all services and resources provided by Sentara and available within the community. Act as a resource for patient and referral source issues.
Requirements
- Trade School Graduate
- LPN License
- 1 year of clinical experience
- 2 years of Customer Service experience
