Clinical Records Coordinator - Full Time Schedule: Monday-Friday 8am-5pm Pay: $22.95 - $29.07 / hour DOE What you’ll be doing: You will be responsible for the completeness, maintenance, and security of medical records in compliance with policy and State and Federal guidelines and regulations. You will also assess for completion and timeliness of documentation by nursing staff and IDT. Will participate as requested in implementing corrective action, and educating clinical staff. Implement and maintain systems for safe, secure, and accurate storage and retrieval of health information and data in compliance with state and federal guidelines as well as policies and procedures. Manage computerized and hardcopy medical records through accurate and timely scanning, efficient retrieval, proper retention and destruction of records, maintenance and update of resident files and census information. Develop a routine system for scanning paper documents into electronic records to minimize separate paper charts. Monitor and control the release of health records for your facility by upholding policies, PRS standards of practice, and HIPAA/legal regulations. Develop a thorough understanding of electronic medical record computer systems and become a systems super-user for community. Monitor security of systems, follow up with possible breaches in security or privacy. Perform regular reviews and audits of resident charts, daily and monthly census sheets, medication and treatment charting, CNA charting, admission and discharge records, physicians’ orders, and Service Plan records. Support Medicare compliance through monitoring and auditing for presence and accuracy of Medicare notices and Certs/Recerts, coding and physician visits. Report these findings in monthly QAPI meetings along with any other audit results. May be asked to participate in Process Improvement Projects (PIP). Compile and analyze facility statistical reports for auditing purposes, administration, and regulatory agencies. Run reports through electronic health records as requested by IDT. May be assigned as confidentiality/privacy officer, work with administration to implement privacy compliance programs and understand corporate privacy policies and procedures. Design programs to improve the quality of documentation by clinical staff, make process improvement recommendations to the quality improvement committee, and facilitate education on medical records and documentation for facility staff. May be asked to attend and participate in Utilization Review Committees. Demonstrate proficiency in Diagnostic Coding. Participation in Triple Check meetings. Prepare for and attend daily Stand-Up meetings. Conduct checks to insure Disaster Recovery Computer/Printer is functioning at all times. Perform functions of the job in a competent, timely and effective manner that ensures maximum productivity and a high level of resident care. Staffing, payroll support, and supply management. Perform other duties as assigned.
