About The Position

Provides oversight, consultation, research, management, and education in home care. Performs data analysis, identification of areas for improvement, action planning, implementation, and ongoing monitoring of results. Monitors data and plans for risk mitigation of infection prevention risks specific to home care.

Requirements

  • Requires a strong understanding of concepts, practices, and procedures related to process improvement, human factors, safety principles, and change management.
  • A minimum of 3 years' relevant experience required
  • Bachelor's degree or higher in Nursing required upon hire OR Bachelor's degree or higher in related field required upon hire
  • RN - Licensed as a Registered Nurse. Eligible to practice nursing in the State of Kentucky required upon hire
  • BLS - Certified as a Basic Life Support Healthcare Provider required within 45 days of employment in this position

Responsibilities

  • Provides direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization.
  • Monitors current best practices and local, state, and federal regulatory requirements specific to home care.
  • Serves as an expert consultant for staff in areas of program development, clinical practice, and professional development.
  • Coordinates learning activities, in collaboration with the home care educator, to improve staff knowledge and compliance with best practices and regulatory requirements.
  • Contributes to defining, directing, and evaluating the nursing care and functions of clinical staff.
  • Audits the electronic medical records of home care patients for oversight and compliance.
  • Manages improvement projects in support of best practice and disseminates findings while ensuring follow-up is completed.
  • Develops, implements, and evaluates educational plans/programs, in collaboration with the home care educator, and utilizes methods, measurements, and tools to evaluate achievement of learning goals.
  • Coordinates activities and projects to meet organizational and regulatory agency goals.
  • Maintains awareness of current trends, practice changes, and research findings related to home care.
  • Collects and performs statistical analysis, interpretation, and dissemination of data related to program, including reporting of data and results of interventions up through the organization structure.
  • Performs job duties in accordance with the Nurse Practice Act.
  • Monitors infection prevention data, in collaboration with the infection preventionist, to identify trends, and implement and evaluate action plans to mitigate infection risk.
  • Coordinates emergency management drills or post-event analyses specific to home care, in collaboration with the emergency management organization leaders, to meet organization, local, state, and federal regulations.
  • Utilizes the plan of care to monitor patient progress and assure patient requirements are matched with appropriate resources.
  • Facilitates the progress and outcome of the patient's plan of care.
