About The Position

The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. This individual interfaces directly with Mount Sinai Health System Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice

Requirements

  • Baccalaureate Degree in Nursing is required.
  • Master’s in Nursing is required
  • Progressive job related experience specific to area of responsibility
  • New York State License with current registration as a registered professional nurse, nurse practitioner preferred.

Responsibilities

  • Responsible For Development, implementation, coordination and evaluation of clinical programs and services related to core measures.
  • In conjunction with Nursing, Medical and Quality Leadership, coordinates core measures and clinical intervention compliance activities.
  • Assists in development and design of action plans to support compliance with measures indicators and provides support and educational resources to departments and services relative to core measures quality assessment and improvement processes.
  • Assists in establishing and implementing short and long term goals, objectives, policies and operating procedures for core measures programs.
  • Provides coordination and leadership in the execution of day to day program activities, as appropriate to program objectives and areas of expertise.
  • Oversees the collection, compilation, and analysis of clinical program activity data.
  • Provides technical assistance, resource services, consultation and education to clinical departments relative to core measures activities.
  • Participates in and conducts process improvement analysis where applicable.
  • Responsible for extension of the program in terms of new services or areas.
  • Promotes an interdisciplinary approach in patient care delivery.
  • Serves as a resource for patients and their families.
  • Acts as a patient advocate utilizing the Patient Bill of Rights.
  • Participates in promoting and maintaining a safe and therapeutic environment.
  • Supports the mission, philosophy, goals and objectives of the Department of Nursing and The Mount Sinai Medical Center.
  • Maintains clinical competence.
  • Is responsible and accountable for his/her own nursing practice.
  • Advocates the highest standards of nursing practice.
  • Approaches patients, families and other members of the health care team in a professional, respectful manner.
  • Serves as a clinical role model and resource to the nursing staff.
  • Serves as a professional resource to other members of the health care team.
  • Demonstrates professional involvement by: a. Attending and participating in workshops, seminars, and courses as presenter or author. b. Keeping abreast of current literature and trends in practice. c. Participating in related professional organizations.
  • Considers ethical issues in professional nursing practice.
  • Treats co-workers, patients and families with dignity and respect.
  • Is open and responsive to the diverse backgrounds and experience of other people, and promotes and environment that is sensitive to cultural diversity.
  • Maintains attendance and punctuality record in accordance with departmental standards.
  • Adheres to dress code in accordance with departmental policy.
  • Prepares reports and analysis associated with the institutions core measures programs i.e. senior management reports, departmental and clinical service program evaluation, quarterly reports, etc.
  • Facilitates cooperation within the program between all departments, services and units.
  • Provides timely status reports of results of quality monitoring relative to areas that are assigned including identification of patterns and trends that require actions.
  • Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations.
  • Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Assures readiness for Joint Commission and other agency surveys and presents related programs during Joint Commission and other agency visits.
  • Participates in the development and implementation of continuous quality improvement programs.
  • Participates in quality management activities including identifying and communicating issues of performance improvement and risk in a timely manner.
  • Participates in committee activities.
  • Is aware of and responsive to budgetary constraints and incorporates financial principles into program development and allocation of resources.
  • Recognizes each individual's right to health care unrestricted by age, race, color, sex, age, national origin, disability, sexual orientation or source of payment.
  • Assists in the performance appraisal process of designated Nursing staff.
  • Researches opportunities for use in benchmarking for assigned areas.
