About The Position

The Clinical Program Coordinator-Stroke is responsible for programmatic coordination of clinical, regulatory and performance improvement activities related to the stroke patient population. Under the leadership of the Network Stroke System of Care Manager and in collaboration with the Stroke Medical Director, the Stroke Program Coordinator is accountable for implementation of the Comprehensive Stroke Center's program goals. This includes developing and maintaining systems that ensure clinical, operational and service quality goals have a process in place to achieve the relevant standards. Successful execution of this role is instrumental in the continued development and implementation of the organization's stroke accreditation program. This includes compliance with The Joint Commission (TJC) Accreditation Guidelines, CMS Conditions of Participation and other state and federally mandated regulatory agency guidelines as appropriate. This position also functions as the stroke system coordinator for activities relating to patient and family education, promotion of continuity of care, community education, staff education, and committee work related to the stroke program. Key to this position are data collection, analysis and reporting for the purpose of advancing the stroke program's clinical and academic goals.

Requirements

  • State of Vermont Registered Nurse (RN) license required.
  • Five years of relevant nursing experience.

Nice To Haves

  • Master's in related field considered strongly.
  • Stroke certification preferred.
