The Clinical Preceptor – Nursing serves as a key leader in shaping clinical excellence through hands-on education, mentorship, and direct patient care. This role supports the onboarding and continued development of nursing staff by facilitating clinical orientation, validating core competencies, and coordinating observation experiences for nursing students. Utilizing evidence-based practices, adult learning principles, and strong coaching skills, the Clinical Preceptor fosters professional growth in a supportive and collaborative environment. In addition to guiding learners, the Preceptor maintains an active patient care role—managing a diverse, community-based caseload and modeling high standards of nursing practice. Requirements

  • Graduate of State Board approved program for Registered Nurses
  • Valid driver's license when driving any vehical for work-related reasons.
  • Auto insurance.
  • Current license to practice nursing according to state where service is provided.
  • COS-C certification must be obtained within 12 months of hire.
  • Valid Mandatory Reporter course completion by state requirement.
  • BLS
  • 1 year of home health or hospice case managment.
  • Use of usual + customary equipmen used to perform essential functions of the position.

Responsibilities

  • Facilitates coordinated transitions from orientation program to independence within targeted goal.
  • Tracks required orientation progress and provides progress to the Clinical Supervisor
  • Demonstrates competency and proficiency in case management for provision of care, clinical expertise, documentation, utilization effectiveness, patient experience and quality outcomes.
  • Completes all documentation accurately and submits to office within 24 hours of visit, phone call and/or change in treatment or orders.
  • Makes recommendations related to training and skills development to clinical education team.
