The Clinical Pharmacy Technician plays an important role in protecting our patients from medication errors caused by inaccurate medication histories. Under the supervision of a pharmacist, the Clinical Pharmacy Technician will function in the critical role of obtaining an accurate medication history. This will include interviewing patients, utilizing software and contacting their outpatient pharmacies/facilities. The home medication list compiled by the Clinical Pharmacy Technician will have any discrepancies reconciled by a registered pharmacist to ensure patient safety and optimize pharmacotherapy. The hours for this position will alternate between the hours of 10am and 8:30pm to include every 3rd weekend.

  • Obtains accurate medication histories from patients in the Emergency Department or inpatient units as necessary
  • Interviews patients/patient's caregivers to obtain most precise data about the patients medications and supplements (e.g. name of medication, dose, frequency, route, last dose taken, last time refilled)
  • Calls patient's outpatient pharmacy and or healthcare facility to obtain the best possible medication history
  • Utilizes electronic health record to collect patient related information and document work done as necessary
  • Communicates all medication discrepancies or complicated medication regimens to registered pharmacist
  • Utilizes available resources to communicate with a translator if necessary
  • Communicates with other healthcare professionals as necessary
  • Demonstrates an understanding of and adheres to all medication use and pharmacy policies and procedures
  • Performs other pharmacy technician duties as assigned
  • If Outpatient Pharmacy Technician is qualified, enter prescriptions into the order entry system
  • Other duties and/or projects as assigned
  • Adheres to HMH Organizational competencies and standards of behavior
  • High School diploma, general equivalency diploma (GED), and/or GED equivalent programs
  • At least one year of pharmacy experience (outpatient or inpatient) required
  • Effective oral and written English communication
  • Strong ability to multitask
  • Ability and willingness to work cooperatively with others
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms
  • PTCB Medication History Certification - uncertified candidates are strongly encouraged to pursue certification after employment
