This role is under the direct supervision of the Clinical Pharmacy Coordinator of Specialties/Population Health Management, performs a wide range of routine and complex duties in support of clinical pharmacy, with a focus on optimizing patient medication adherence and health processes and outcomes as reflected by quality-of-care measures. Responsibilities include but are not limited to, registry management, chart review, patient outreach, meetings with payors, and optimization of workflows to help the organization maximize opportunities with payor star measures. As a Clinical Pharmacy Technician, you will perform live telephonic and electronic outreach to Reliant Medical Group patients & members to improve health quality outcomes (i.e. medication adherence, appropriate drug utilization).   • Reviews patient registries to implement a plan for outreach to patients flagged with gaps in their care, including a particular focus on non-adherence to medications
  • Attend meetings with payors as delegated to optimize patient lists and assist in report out of information and workflows
  • Manage patient lists to identify and contact patients who have a history of lack of adherence to medication care plan or have missed expected medication fills
  • Monitors and follows up with patient care activities under the direction of Clinical Pharmacy Coordinator/Clinical Pharmacists
  • Reviews and handles telephonic or electronic messages to patients to optimize adherence to medication and care plan, and triages to appropriate individuals as needed
  • Processes prescription renewals as needed
  • Contacts patients and/or reviews of medical or pharmacy records for prescription information, medication adherence, drug allergies, medication review, laboratory testing and results, and patient medication education
  • Extracts pharmacy benefit management information for department staff and for patient-specific needs
  • May prepare regular reports for supervisor
  • May perform further support to the department with tasks such as answering phones, ordering supplies/demos, supporting clinic scheduling and assisting with other patient forms
  • Performs other certified pharmacy technician duties for the department as assigned by supervisor
  • Able to use all electronic tools and applications relevant to the performance of the duties of the position
  • Performs all job functions in compliance with applicable federal, state, local and company policies and procedures
  • Accesses only the minimum necessary protected health information (PHI) for the performance of job duties
  • Comply with all Atrius Health and Reliant Medical Group and departmental privacy policies, procedures and protocols
  • High School Diploma/GED (or higher)
  • Active & unrestricted Pharmacy Technician license in the state of Massachusetts
  • National Pharmacy Certification (PTCB, CPhT)
  • 3+ years of Pharmacy Technician experience
  • Intermediate level of proficiency with MS Office
  • Must reside in the state of Massachusetts
  • Previous EPIC knowledge/experience
  • Retail and/or hospital pharmacy experience
  • Knowledge of medication abbreviation and symbols as well as common drug nomenclature
  • Knowledge of and previous experience with laws and regulations related to pharmacy practice and patient confidentiality
