Administration of Human Resource Programs

This Patient Aligned Care Team (PACT) Clinical Pharmacist Practitioner (CPP) functions at the highest level of clinical practice - working independently under their scope of practice within the primary care setting - as defined by the individual medical center to directly care for patients. This position is located at our West Plains - MO CBOC.

  • Coordinates, develops, manages and controls chronic disease state management clinical pharmacy services.
  • Provides chronic disease state management clinical pharmacy services to PACT patients through consultations, face-to-face visits, telephone visits, Clinical Video Telehealth (CVT), VA Video Connect (VVC), Care Coordination Home Telehealth (CCHT), Group Education Visits/Shared Medical Appointments, and secure messaging.
  • Provides input to Associate Chief of Pharmacy - Clinical Services and Chief of Pharmacy in establishing and accomplishing the pharmacy's missions and goals.
  • Initiates, modifies and monitors pharmaceutical care plans pertaining to medical conditions managed in the primary care setting.
  • Assesses the appropriateness and effectiveness of patients' medication regimens by reviewing medical records and interviewing/examining the patients.
  • Makes direct changes to medication regimen, monitors and assesses therapeutic and adverse effects of drug therapy, and orders lab tests as necessary.
  • Educates patients and/or caregivers regarding patients' medical conditions and on proper use and care of their medications.
  • Serves as a consultant to other providers for intensive medication therapy management services.
  • Performs and documents medication profile reviews as mandated by program handbook.
  • Citizen of the United States.
  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
  • Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States, or the District of Columbia.
  • One year of experience equivalent to the next lower grade level (GS-12).
  • Completion of an American Society of Health-System Pharmacists (ASHP) accredited residency program.
  • Active and current Board of Pharmacy Specialties (BPS) certification in Pharmacotherapy or Ambulatory Care.
  • Completion of an ASHP accredited Post-Graduate Year 2 (PGY-2) residency program.
