Responsible for providing safe, effective, appropriate and cost-efficient medication therapy. Ensure that pharmacy staff and, when applicable, medical students and residents, receive ongoing training andpetency assessment, and that they participate in patient medication reconciliation and performance improvement activities related to clinical pharmacy services. Contribute to development and maintenance of an effective formulary based on safety, efficacy and cost. Work with physicians to develop/improve clinical treatment guidelines.
