In this role, you will provide medication management through a thorough evaluation of medications for patients, ensuring safe and effective utilization tailored to specific medical conditions. You will collaborate with patients, providers, nurses, and other healthcare professionals to facilitate access to specialty medications and ensure optimal patient outcomes. Our approach to patient care is team-oriented, and we support each other to complete necessary patient care responsibilities daily. This position is exempt and offers the potential for a hybrid work model, allowing for both on-site and remote work.
