About The Position

The Central Ohio VA Healthcare System is recruiting for a Clinical Pharmacy Specialist - PACT Clinical Pharmacy Practitioner to be located in the Zanesville Community Based Outpatient Clinic. This position will function in Pharmacy Service and serves as a consultant to physicians and other health care providers in the facility.

Nice To Haves

  • Experience in primary care chronic disease management.
  • Experience in direct patient care and comprehensive medication management responsibilities.
  • Advanced pharmacy degree (PharmD), board certification (BPS), PGY1 Pharmacy Practice Residency or equivalent experience.

Responsibilities

  • Functions independently under his/her approved scope of practice, as a mid-level provider to initiate, modify or discontinue medication therapy, and as a consultant for intensive medication therapy management services.
  • Helps achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas.
  • Performs physical assessments related to monitoring of drug therapy.
  • Orders laboratory and other tests (ex. ECG) to help evaluate efficacy of medication therapy.
  • Participates in the Pharmacy Service and facility quality improvement processes.
  • Assists in the development of proposals for new clinical pharmacy services.
  • Serves as a resource to the facility for matters related to the TJC and other regulatory agencies.
  • Attends, participates and contributes as an active member of facility committees and/or VISN committees as assigned/appointed.
  • Participates in Quality Assurance (QA) activities including Medication Use Evaluation, Root Cause Analysis (RCA), and Healthcare Failure Mode and Effects Analysis(HFMEA) activities when requested/assigned by supervisor.
  • Responds to healthcare team medication information questions.
  • Precepts and evaluates pharmacy students and pharmacy residents.
  • Serves as a member of the pharmacy residency team including attending pharmacy residency team meetings.
  • Participates in the evaluation and administration of the Residency Learning System (RLS) as outlined by the American Society of Health-Systems Pharmacists (ASHP).
  • Assists pharmacy residents with conducting research projects.
  • Provides pharmacy educational programs such as in-services, continuing education, journal club, and classes to patients and healthcare staff.
  • Educates patients, family members, or caregivers on the proper use and storage of medications through interactions and the use of medication profiles.
