Administration of Human Resource Programs

The Clinical Pharmacy Specialist (Nephrology) will be assigned to Nephrology clinics and the hemodialysis section of renal service. The Clinical Pharmacy Specialist functions as a member of their assigned team(s) and serves as an advanced practice provider. to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services.

  • Executing therapeutic plans utilizing the safest and most cost-effective medication treatments.
  • Ordering, subsequent review, and action on appropriate laboratory tests and other diagnostic studies necessary to monitor, support, and modify the patient's drug therapy.
  • Ordering consults and referrals, as appropriate, to maximize positive drug therapy outcomes; examples include, but are not limited to, referrals for routine care or assessment to clinical nutrition professionals, prosthetics, social workers, integrated mental health professionals, or other specialty areas.
  • Conducts virtual and in-person (when appropriate) encounters with patients for CMM in the Nephrology and hemodialysis practice areas.
  • Follows-up with patients in a timely manner and monitors for drug therapy response, potential adverse drug reactions, allergies, compliance, laboratory results, and other monitoring measures as appropriate; treatment plan adjustments are made accordingly based on assessed measures.
  • Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, pharmacokinetic impact, and therapeutic outcomes as required; information is communicated with team members and appropriate alternatives to current treatment plans are provided.
  • Conducts medication utilization reviews, quality assurance activities, and utilization reviews as required.
  • Instructs patients and family members effectively in the appropriate use of medications and medical devices, adjusts communication and tracking methods based on age and developmental considerations, accurately assesses and documents patient comprehension, and refers patients to available educational programs.
  • Collaborates in the development and maintenance of policies and procedures defining operational requirements for the clinical pharmacy program.
  • Adheres to and ensures the pharmacy service is compliant with all regulatory and accrediting body requirements, as well as renal/hemodialysis related policies and procedures.
  • Serves as a preceptor for pharmacy students and pharmacy residents in accordance with established policies.
  • Attends and actively participates in pharmacy related training, in-services, staff development, and relevant conferences.
  • Serves as an advocate for the pharmacy profession and educates team members.
  • Assists in the training of new CPS, competency assessments, and mentoring within the area of practice as requested.
  • Responsible for keeping abreast of changes in the Pharmacy Benefits Management (PBM), CPPO, and PBM Formulary services (e.g., formulary additions and deletions, relevant memoranda, and policies and procedures), as well as general knowledge of VA, Drug Enforcement Administration, US Food and Drug Administration, and The Joint Commission regulations.
