Essential Job Responsibilities Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values Knowledgeable in medication therapy management Exhibits clinical and professional skills to positively interact with patients & all members of the healthcare team Follows institutional medication policies and procedures as well as state and federal laws regarding medications. Works with inpatient and outpatient healthcare team to optimize medication therapy and improve medication accuracy during transitions of care. Accurately interprets medication orders in the computer for transitions of care and drug administration. Contacts the provider in a timely manner when there is a concern about a medication management. Provides drug information, drug therapy recommendations, and education to physicians, nurses, patients, pharmacy staff, residents and students related to transitions in care, chronic disease state management, and internal medicine. Reports adverse drug events and near misses in the medication management process across the continuum of care. Ensures continuity of care through evidenced based methods, including, but not limited to, medication reconciliation, self-management plan, engagement of family and care givers, education and referrals through all phases of patient care. Creates population-based management strategies and processes for medication therapy management that help patients manage their healthcare needs and foster care quality, cost-effectiveness, and patient engagement. Works with interdisciplinary team to develop care plan and address gaps in care. Works closely with patient to understand disease specific related issues or interpret health risk assessment information to ensure patient meets or exceeds goals. Promotes and facilitates improved clinical outcomes and patient satisfaction, as well as efficient use of resources in order to obtain optimum value for both the patient and reimbursement. Establishes collaborative partnerships with patients to assist them in examining patterns of health care needs, decisions, lifestyle choices, and utilization of resources that affect their health. Participates in organizational committees related to medication therapy management. Reviews appropriate use of selected medications to include optimization of use and cost considerations. Contributes to the continued development of the practice of the profession of pharmacy. Additional responsibilities and expectations of the TOC/Opioid Stewardship Pharmacist: A. Assists with policy and protocol development related to pain management and opioid use disorder B.
