Requirements

  • PharmD or equivalent experience from an accredited pharmacy school.
  • Completion of an ASHP-accredited PGY-1 Pharmacy Residency.
  • Active license from the Texas Board of Pharmacy.
  • Certification in Basic Life Support, First Aid, and CPR.
  • Proficiency in computer applications, including Windows, Microsoft Word, and Excel.
  • Ability to manage multiple priorities and deadlines effectively.
  • Excellent communication and customer service skills, with a strong sense of professional discretion.

Nice To Haves

  • Board Certification as a Pharmacotherapy Specialist or Ambulatory Care Pharmacist.
  • At least 2 years of community pharmacy and medication adherence counseling experience preferred.
  • Experience in specialty areas such as HIV/AIDS, geriatrics, or pediatrics is advantageous.
  • In-depth knowledge of advanced pharmacy practices and evidence-based medicine.

Responsibilities

  • Deliver pharmaceutical care services through direct, innovative interactions with patients and healthcare providers.
  • Conduct both face-to-face and virtual consultations to manage chronic disease states with cutting-edge medication management.
  • Utilize advanced techniques to obtain and analyze comprehensive medication histories for optimized patient care.
  • Reconcile patient medications during ambulatory care visits, ensuring safety and efficacy.
  • Engage in physical assessments and patient triaging as necessary, employing the latest diagnostic tools.
  • Document encounters and interventions meticulously within an electronic medical record system.
  • Design, implement, and assess therapeutic plans, utilizing state-of-the-art, evidence-based practices.
  • Manage medication therapy, including initiation and modification, within approved procedural guidelines.
  • Order and interpret laboratory tests to monitor and enhance medication efficacy and safety.
  • Report and document adverse drug events meticulously, adhering to best practices and regulatory standards.
  • Provide comprehensive drug information and education, promoting informed decision-making among patients and providers.
  • Mentor pharmacy students and residents, fostering the next generation of healthcare innovators.
  • Adapt quickly to evolving healthcare demands, contributing to a culture of continuous improvement.
