The primary purpose of this position is to provide specialized integrated clinical pharmacy services for an assigned area and to provide leadership in education and practice of evidence-based medication use and optimal patient outcomes.

  • Evaluates, monitors, &/or modifies patient pharmacotherapy to ensure effective and safe patient care.
  • Designs, implements, and manages patient drug regimens based on evidence-based medicine, formulary compliance, and individual patient needs.
  • Demonstrates competency/knowledge in computer entry/verification of medication orders/order processing/order preparation including specialty orders.
  • Participates in the development, management, education, & implementation of cost-effective, evidence-based therapies or protocols.
  • Actively participates in departmental &/or hospital committees/meetings as medication management authority in specialized practice area.
  • Educates and communicates effectively with patients on medication therapies.
  • Provides a lead role in answering complex drug information requests.
  • Provides mentoring/training/education to pharmacy students, residents, pharmacists, technicians.
  • Attends, participates, & provides feedback in journal club, in-services, resident topic discussions, and resident/student case presentations.
  • Documents interventions for resolved prescribing, formulary & therapeutic issues.
  • Maintains a positive working relationship with job-related contacts.
  • Maintains knowledge of applicable rules, regulations, policies, laws and other guidelines that impact the Pharmacy profession.
  • Stays abreast of the latest developments, advancements, and trends in the field of pharmacy & medicine.
  • Conducts/participates in research which impacts delivery of quality care.
  • Must have a Bachelor's Degree in Pharmacy or Pharm. D. degree from an accredited school of pharmacy.
  • Successful completion of an ASHP-accredited post-graduate year one (PGY1) residency training program and post-graduate year two (PGY2) residency training program or fellowship.
  • Meets Parkland standards of an ASHP-Qualified Residency Preceptor within one year of employment.
  • Texas Pharmacist License or eligible for Texas Pharmacist License; must obtain licensure within 90 days of employment.
  • Must demonstrate competency and ability to perform in the desired practice area.
  • Must possess knowledge of drugs, procedure, equipment, common disease states and patient care monitoring within the assigned care areas.
  • Must possess good oral and written communication skills.
  • Must possess good organizational skills.
  • Must have proficient computer skills.
  • Must possess extensive mathematical skills.
  • Prior experience working in assigned practice area preferred.
  • Prefer national certification in any Pharmacy specialty such as Board of Pharmacy Specialties (BPS) certifications.
