About The Position

The Ambulatory Care Clinical Pharmacist, specializing in Infectious Disease at Mercy Health, is responsible for optimizing drug therapy outcomes by providing high-quality, cost-effective healthcare. This includes influencing prescribing behaviors, identifying, preventing, and resolving drug-related problems, and educating healthcare personnel on drug therapy. The role involves active participation in clinical programs such as in-person or telephonic pharmaceutical care clinics, therapeutic drug monitoring programs, and consult agreements. Key responsibilities include managing OPAT patients, rounding with RN/MD teams, assisting with HIV and PrEP counseling and medication management, aiding in therapeutic drug monitoring for long-term antifungals, and organizing PGY1 ID rotations. The specialist delivers patient care using the nursing process, coordinating patient teaching, and directing care provided by health team members. This position requires consistent participation in clinical programs and initiatives (>90% of the time) and ensuring safe, rational, and cost-effective therapy while complying with all applicable federal, state, and accrediting agency regulations and departmental policies.

Requirements

  • Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD
  • Maintains current license for pharmacist in the state(s) where practicing
  • Valid state issued ID

Responsibilities

  • Optimize drug therapy outcomes by providing high quality, cost-effective health care

  • Identify, prevent and resolve drug-related problems
  • Educate health care personnel regarding drug therapy
  • Participate in clinical programs to improve patient care, including but not limited to participating in in-person or telephonic pharmaceutical care clinics, therapeutic drug monitoring programs, participate in consult agreements, and programs to support state-wide initiatives
  • Manage OPAT patients; responsibilities include completing a referral note within 48 hours of hospital discharge, monitoring weekly (or 2x weekly) labs, managing side effects, aiding in transitions of care concerns, following clinical progress, and determining plan of care at end of therapy
  • Round with each RN/MD team to review OPAT list
  • Aid in HIV and PrEP counseling and medication management (including prior auths and insurance concerns where needed)
  • Aid in TDM for long term antifungals
  • Organize and facilitate PGY1 ID rotation for The Jewish Hospital – Mercy Health’s Residency Program
  • Answer any drug information questions from MDs, RNs, inpatient pharmacists etc.
  • Deliver patient care utilizing the nursing process through data collection and assessment
  • Identify and determine the priority of patient’s problems/needs
  • Evaluate the process and outcome of nursing care
  • Coordinate, provide, and direct patient teaching activities
  • Coordinate the care provided by health team members
  • Participate in clinical programs and initiatives >90% of the time with a consistency in schedule
  • Ensure safe, rational, and cost-effective therapy in their respective practice area
  • Act in compliance with all applicable rules, regulations, and standards as promulgated by Federal, State, and accrediting agencies or regulating bodies
  • Maintain established departmental policies and procedures while promoting department objectives
  • In collaboration with the interdisciplinary care team, conduct initial and ongoing patient assessment, analyze assessment data, create a plan of care, implement treatment and evaluate treatment effectiveness
  • Administer medications in a safe manner consistent with the State of Practice and Bon Secours Mercy Health policies and procedures
  • Serve as point of contact for patients' care coordination throughout hospital departments
  • Act as a patient safety advocate by participating in ongoing quality improvement in the department
  • Incorporate the professional practice model into care delivery
  • Provide comprehensive pharmaceutical care to a focused population and serve as a subject matter expert in area of specialty
