The position involves providing clinical pharmacy services to patients undergoing treatment for malignant hematology, medical oncology, gynecologic oncology, and stem cell therapy (SCT). The pharmacist will work closely with pediatric pharmacists for pediatric oncology and will be responsible for identifying, solving, and preventing medication-related problems in the hematology/oncology/SCT population. The role includes educating healthcare teams, patients, and families about the safe and effective use of medications, particularly in the post-allogeneic SCT population, and facilitating chemotherapy processing for hospitalized patients.

  • Participate in interdisciplinary rounds for malignant hematology, medical oncology, gynecologic oncology, and SCT daily.
  • Review drug therapy to ensure appropriate use, dose, dosage form, regimen, route, therapeutic duplication, drug allergies, drug interactions, and cost-effective therapy.
  • Provide individual patient/family counseling on medication regimens and adherence prior to discharge.
  • Participate in discharge planning and discuss relevant changes to the home medication list.
  • Act as the pharmacy liaison to the Cancer Center and coordinate pharmacy participation in related committees.
  • Participate in quality improvement initiatives and outcomes-based research for hematology, medical oncology, gynecologic oncology, and SCT programs.
  • Develop, implement, and monitor hematology/oncology/SCT-related clinical protocols and guidelines.
  • Bachelor of Science in Pharmacy.
  • Illinois Pharmacist License (or eligibility to be licensed in Illinois).
  • Post Graduate ASHP Year Two Pharmacy Residency in Hematology/Oncology.
  • Board Certification in Oncology Pharmacy Specialty (BCOP) prior to or within 18 months of hire.
  • BLS Certification.
  • Knowledge in intravenous admixture services, intravenous drug therapy, and chemotherapy.
  • Excellent problem-solving skills and ability to work independently.
  • ACLS Certification (or shall be obtained upon hire).
  • Doctor of Pharmacy degree and residency certificate.
  • Experience and strong knowledge base in chemotherapy.
  • Competitive salary range of $63.23 - $99.59 per hour.
