About The Position

This pharmacist position provides pharmaceutical care services for ambulatory patients of Washington County Hospitals & Clinics. This pharmacist aids as a practitioner-educator to providers, peer pharmacists, students, and other patient-facing healthcare staff. Pharmacotherapy and disease state focuses include, but are not limited to, type 2 diabetes mellitus, hypertension, anticoagulation, obesity, polypharmacy, and geriatric care. To perform this job successfully, an individual must be able to perform each responsibility satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability to be successful.

Requirements

  • Degree in Pharmacy (PharmD) from an accredited college and State of Iowa licensure to practice pharmacy and to dispense controlled substances.
  • Certified to administer vaccines in the state of Iowa.
  • BLS certification or obtain within 30 days of hire
  • Authorized or qualified for authorization as a preceptor of student interns in Pharmacy as described by the State Board of Pharmacy. Authorization to be a preceptor requires two years of practice as a pharmacist.
  • Strong written and verbal communication skills with prevalent professional demeanor
  • Ability to handle sensitive and confidential situations and documentation
  • Well-developed interpersonal skills
  • Analytical skills to identify and resolve problems in a timely manner
  • Maintain professional technical knowledge
  • Maintain current knowledge of State and Federal regulations and statutes regarding the practice of pharmacy and the distribution of approved drugs, controlled substances and investigational agents

Nice To Haves

  • Completion of an ASHP-accredited PGY1 Pharmacy Practice Residency AND PGY2 Ambulatory Care Pharmacy Residency Preferred

Responsibilities

  • Be present in clinics when patients are scheduled to see the clinical pharmacist and when clinical pharmacy services are required.
  • Complete a comprehensive patient assessment including medication history, pertinent physical assessment, assessment of relevant laboratory values, patient interview, and an evaluation of past medical history for appropriate patients.
  • Collaborate with physicians, nurses, pharmacists, other health care providers, and patients as a member of the interprofessional team to achieve positive outcomes through optimal medication use and management.
  • Provide in-depth patient medication assessment and education
  • Provide follow-up of therapeutic recommendations through telephone and return clinic appointments as appropriate.
