About The Position
The Clinical Pharmacist Specialist is responsible for providing safe, effective, and timely medication therapy management. In this role, they support both centralized and decentralized medication-use systems, ensuring optimal medication therapy for patients with various medical conditions. The Clinical Pharmacist Specialist works within a decentralized practice setting alongside physicians, nurses, and other healthcare professionals to deliver patient-centered care. These specialists are involved in interdisciplinary programs and specialty services, managing medication therapy for specialty patient care and ensuring integration of pharmaceutical care across the institution.
