This Clinical Pharmacist will provide a broad range of professional and clinical pharmacy services in a geographically vast - highly rural medical center. The incumbent will be responsible for the pharmacy care in the Community Based Outpatient Clinics (CBOCs) including but not limited to Community Care - the Non-VA Care - and the Veterans Choice programs.

  • Ensures that all Pharmacy Service and other regulatory policies are complied with regarding medication labeling, inventory management, formulary management, equipment maintenance and timeliness of service.
  • Ensures all prescriptions coming from Non-VA providers are processed, working with the providers, patient, and other services to ensure timely delivery of these services within Pharmacy Service Policy.
  • Evaluates medication orders and medication profiles for appropriateness of therapy, dosage range, synergistic, antagonistic, and cumulative effects.
  • Provides consultation and education to physician, nurses, and other health care professionals on drug information.
  • Evaluates provider's orders for appropriateness based on patient medication profiles, inputs information into computer system, dispenses medication in accordance with appropriate formulary requirements.
  • Works with Pharmacy Service procurement employees to ensure adequate stock and avoidance of out of stock situations.
  • Prioritizes work to meet operational and patient care needs.
  • Ensures the appropriate utilization, maintenance, and care of Pharmacy Service resources.
  • Assists customers at the pharmacy window and answers the telephone.
  • Monitors and analyzes orders and distribution patterns to identify questionable circumstances or unusual trends that may suggest diversion or increased usage.
  • Creates an environment to stimulate, encourage and support the participation of pharmacy personnel in providing drug information to the hospital staff and other facilities as deemed appropriate.
  • Attends and actively participates in pharmacy activities, i.e., staff development, pharmacy meetings, etc.
  • Provides a framework for continual learning for pharmacy students, residents, and employees of all levels to ensure competence and improve performance.
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Pharmacists must be proficient in spoken and written English.
  • Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
  • Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
  • 1 year of experience equivalent to the next lower grade level, or completion of an ACPE-accredited Pharm.D. program.
  • Knowledge of professional pharmacy practice.
  • Ability to communicate orally and in writing to both patients and health care staff.
  • Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
  • Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
