Clinical Pharmacist – Noble Community Clinics Manitowoc/Sheboygan Full-Time - 4 - 10 hour days (Minimum 2 days per week Manitowoc Clinic - the other 2 days either in Manitowoc or Sheboygan) Noble Community Clinics provides compassionate, high-quality health care for all members of our community. About the Role The Clinical Pharmacist is an integral member of our integrated primary care team, providing comprehensive, patient-centered pharmaceutical care. This role focuses on advanced clinical practice, interdisciplinary collaboration, program development, and education, with an emphasis on improving quality, safety, and outcomes for the patients we serve.

Requirements

  • Doctor of Pharmacy (PharmD) degree from an ACPE-accredited College of Pharmacy
  • Current Wisconsin Registered Pharmacist (RPh) license in good standing or willingness to obtain WI licensure.
  • Strong clinical judgment with the ability to work independently within an interdisciplinary team
  • Commitment to patient-centered, evidence-based care
  • Current CPR/BLS certification (or ability to obtain)

Nice To Haves

  • Experience or interest in clinical pharmacy services, primary care, or ambulatory care preferred

Responsibilities

  • Provide direct clinical pharmacy services as part of an integrated care team including medical, dental, behavioral health, and chiropractic providers
  • Coordinate and manage day-to-day pharmaceutical care for patients
  • Review patient records, assess allergies, medications, immunizations, and clinical data to ensure safe and effective therapy
  • Conduct patient interviews and provide education on medication use, side effects, and adherence
  • Make evidence-based, patient-centered medication recommendations and therapeutic interventions
  • Monitor controlled substances, including PDMP review prior to dispensing
  • Prepare, administer, and document vaccinations and injectable medications
  • Identify high-risk patients and implement interventions to improve quality and patient safety
  • Support and enhance clinical pharmacy services and programs
  • Ensure compliance with federal and state pharmacy regulations
  • Maintain accurate medication and controlled substance records
  • Oversee pharmacy inventory and equipment functionality
  • Train, mentor, and supervise pharmacy technicians, intern pharmacists, and Doctor of Pharmacy students
