About The Position

We are seeking a Clinical Pharmacist to join our multidisciplinary care team at an upcoming South Tampa location. This role is responsible and accountable for the provision of safe, effective, and evidence-based medication therapy in a decentralized, service-based clinical setting. The Clinical Pharmacist collaborates closely with medical providers, care teams, and operational partners to deliver optimal medication therapy that is timely, cost-effective, and aligned with regulatory, quality, and safety standards.

Requirements

  • Active and unencumbered Florida Pharmacist license
  • Certified to immunize in the State of Florida
  • Completion of an ASHP-accredited PGY-1 residency (required)
  • Minimum of 3–5 years of experience as a Clinical Pharmacist
  • Demonstrated knowledge of advanced pharmacy practice and evidence-based medication therapy management
  • Ability to work effectively with diverse social, economic, and ethnic populations
  • Strong clinical judgment, critical-thinking, and problem-solving skills
  • Excellent verbal and written communication abilities
  • Collaborative mindset and ability to work across interdisciplinary teams
  • Proficiency with electronic health records and Microsoft Office products
  • Commitment to patient-centered care and service excellence

Nice To Haves

  • PGY-2 specialty residency or equivalent experience (preferred)

Responsibilities

  • Lead assigned patient service lines from a clinical and therapeutic perspective
  • Deliver direct patient care as part of a multidisciplinary healthcare team
  • Provide medication therapy management, reconciliation, counseling, and coordination of prescription delivery
  • Identify high-risk patients and implement interventions to improve patient safety, quality, and outcomes
  • Make evidence-based, patient-centered medication recommendations
  • Support specialty medication procurement within service-based programs
  • Develop, implement, and oversee pharmacy-related policies, procedures, and quality initiatives
  • Evaluate opportunities for operational, quality, and financial efficiencies and benchmark against best practices
  • Participate in pharmacy operations and maintain proficiency in clinical and operational programs
  • Support training, education, and experiential learning opportunities for pharmacy learners
  • Ensure compliance with HIPAA, HRSA, State, Federal, and TFHC regulations
