About The Position

Cornell Scott-Hill Health Center is looking to add a Per Diem Clinical Pharmacist to our team. The Clinical Pharmacist is responsible for planning and providing clinical pharmacy services, disease management, Medication Therapy Management and other patient/staff education programs ensuring compliance with current regulations and in accordance with health center needs and national best practices. The Clinical Pharmacist will document outcomes, supervise students and participate in health center committees and initiatives. He/she will assist the Pharmacy Manager in daily pharmacy operations (includes dispensing responsibilities). Per Diem Requirements: The position requires a flexible availability from Monday through Friday 8:30a to 5pm, and Saturday from 8:30 - 12:30pm, with shifts of up to 8 hours per day during the week, as needed. Responsibilities include providing coverage for colleagues' sick days and vacation time. To maintain a Per Diem status, the Clinical Pharmacist must work a minimum of 4 shifts per month, not to exceed 19 hours per week.

Requirements

  • Degree from an accredited college of Pharmacy required.
  • Licensed or eligible to practice pharmacy in the state of Connecticut.
  • Ability to communicate in a professional and effective manner with staff and patients in matters concerning drug therapy is required.

Nice To Haves

  • Residency preferred.
  • Participates in American Society of Health System Pharmacists or other pertinent national pharmacy organization preferred.
  • Experience and familiarity with Joint Commission standards preferred.

Responsibilities

  • Recognizes, communicates and works collaboratively to resolve Drug-Drug and Drug-Food incompatibilities.
  • Designs, implements and supervises evidence-based drug monitoring plans based on provider diagnoses.
  • Identifies, communicates and assists in resolving medication therapy problems.
  • Performs appropriate testing in accordance with CLIA and facility standards.
  • Provides education on medication in the community.
  • Participates in outcome and drug research projects.
  • Assists the Director of Pharmacy with planning and operation of the Pharmacy Department.
  • Assists in training, supervision and evaluation of technicians and pharmacy support staff.
  • Performs all other duties as required.
