About The Position

The Clinical Pharmacist will assist with Senior Medical Director related to patients in the Burbank / Glendale campuses who need further clinical support.  As an example, the Clinical Pharmacist will assist with following up with patients being discharged from an inpatient facility.  The Clinical Pharmacist will assist in managing patients who have higher risk disease states or who are high utilizers of emergency rooms and/or inpatient facilities.  The Clinical Pharmacist may assist in outreaching to providers to coordinate and collaborate medical care for providers.  Will utilize clinical expertise and skills to develop the appropriate clinical criteria/guideline, policy, procedure and clinical judgment to improve clinical care and patient outcomes.  Will promote, assist and collaborate with our providers with clinically appropriate prescribing practices, based on evidence based medicine and national guidelines.

Requirements

  • Graduate of an accredited School of Pharmacy with a current and unrestricted California State Pharmacy License (Pharm.D) required
  • Proficient in Microsoft Applications: Outlook, Word, and Excel
  • Proficient understanding of common pharmacy electronic databases and the ability to type >40 WPM
  • Is accountable, builds trust, collaborates and pursues excellence
  • Has great problem solving skills and “thinks out of the box” to provide solutions
  • Ability to work in a multi-task, fast moving environment, is flexible and adapts well to change
  • Maintain confidentiality and comply with Health Insurance Portability and Accountability Act (HIPAA)

Nice To Haves

  • Knowledge of PBM industry, formulary management, pharmacy claims and managed care principles and practices preferred
  • Minimum of 1 year pharmacy experience within a managed care setting preferred
  • Ability to establish and maintain positive and effective work relationships with all levels internally and externally.

Responsibilities

  • Will be a leader, responsible for supporting the Senior Medical Director to identify patients, create programs and implement clinical initiatives that will improve various performance indicators.
  • Will directly interact with patients and provide education to patients, provide treatment recommendations,
  • Will directly interact with providers throughout the Burbank / Glendale network to provide clinical recommendations that are individualized by patient. Will follow up with and work collaboratively with providers to ensure improved patient care.
  • Will review patient medication profiles and lists to ensure patients are taking medications as prescribed and identifying any medication concerns.
  • Must have robust knowledge of chronic disease/conditions and provide treatment goals, provide disease specific education and improve participation in health care.
  • Interface with Providers, pharmacists, employees.
  • Will assist in reviewing patients to determine if who may benefit from targeted outreach and clinical intervention.
  • Will assist in reviewing readmitted patients to identify any potential improvements in outpatient medical care.
  • Most of patient and provider communication and interaction will be telephonic so must have strong, professional, welcoming verbal communication skills.
  • Responsible for staying current with clinical literature and guidelines.
  • When necessary, travels to provider’s location to create relationships, to support and educate about clinical programs and clinical interventions and support can provide to patients and providers.
  • Performs other duties and responsibilities as assigned.
