About The Position

The Clinical Pharmacist monitors the medication usage for participants to ensure that it is safe, effective, cost efficient and contributes to optimal participant outcomes. In addition, the candidate will be a drug information resource for the medical and nursing teams, participants, and their families. InnovAge is dedicated to empowering seniors to live independently, allowing them to age in their own homes and communities safely. InnovAge offers an alternative to nursing homes through its Program of All-inclusive Care for the Elderly (PACE), which provides enrolled seniors with customized healthcare and social support at PACE Adult Day Health Centers. Requirements

  • Bachelor’s degree from an accredited school of pharmacy with Board Certification in Geriatric Pharmacy (BCGP) or a Pharm D.
  • Current State Board of Pharmacy license required for state of practice
  • Current knowledge and the ability to advise uses of medications, multiple prescriptions, side effects and make potential problems known to physicians.
  • Must have BGCP or if qualifications are met, must obtain BCGP within 30 months of date of hire.

Responsibilities

  • Evaluates medication regimen of participants upon intake and on a periodic basis based on patient’s conditions, and on a semiannual basis and makes the following clinical interventions:
  • Evaluates and reports drug interactions related to other drugs, disease states, pharmacokinetics, and food.
  • Evaluates ppts discussed at IDT for medication related problems
  • Acts as a drug information resource for interdisciplinary team members and clinic staff on ppt specific medication questions
  • Assists the primary care team in maximizing drug therapy outcomes by evaluating aggregated drug-usage reports and performing drug utilization reviews.
  • In collaboration with primary care physician and the IDT team, review participant’s medication list and make recommendations of prescriptions no longer needed due to duplicate drug therapy or inappropriate drug therapy for documented state or other reasons.
  • Alerts primary care to dosing irregularities such as; inappropriate dosing based on disease state, renal function, liver function etc.
  • Monitors response to drug therapy.
  • Optimizes drug therapy against disease states, standards of care and patient goals of care.
  • Reviews dementia patients on psychotropic medications for possible dose reductions and to ensure adequate drug monitoring.
  • Interviews and consults with participants and/or caregiver(s) to determine efficiency and current usage of medications.
  • Performs individualized participant education sessions to answer any medication questions or problems.
  • Instructs participants and or caregiver(s) on the effects of food and alcohol intake, warns of potential side effects and maintains an on-going professional relationship with participants.
  • Assists the primary care team in maximizing drug therapy outcomes by providing education to the team.
  • Specific assigned quality-initiated projects to enhance the overall care of the participants.
  • Participates in the development/implementation of departmental policy and procedures.
  • Participates in on-going team building and team development initiatives.
  • Monitors and informs staff of new drug regulations/therapeutic warnings and ensures that information is understood and being followed according to position held.
  • Presents topics of interest related to drug therapy in the geriatric population to IDT and Center staff
  • Resolves internal and external medication use or access problems and analyzes various operating reports, takes corrective action(s) when needed.
  • Other tasks as assigned.
