As a Clinical Pharmacist, you'll lead assigned patient service lines, clinical areas, and therapeutic programs while delivering direct patient care and clinical practice. Your main responsibilities will include designing and implementing stewardship activities, developing and overseeing policies and procedures for drug purchasing, usage, distribution, and control, and participating in pharmacy operations and medication dispensing. Additionally, you'll maintain proficiency in hospital computer systems and medication ordering systems, and provide accurate, safe, timely, and appropriate medication therapy based on patient age and needs.   • Lead assigned patient service lines, clinical areas, and therapeutic programs
  • Facilitate pharmaceutical care services, direct patient care programs, and medication utilization systems
  • Design and implement stewardship activities and restriction/surveillance programs
  • Track and evaluate assigned pharmacy programs for operational, quality, and financial efficiency
  • Identify practice-related issues and facilitate clinical research, quality improvement, or provider education
  • Develop and oversee policies and procedures for drug purchasing, usage, distribution, and control
  • Assure pharmacy is an integral part of the healthcare delivery system and facilitate enhancement of services
  • Deliver direct patient care and clinical practice, including ambulatory-care and service-based programs
  • Work as an active member of multidisciplinary teams and collaborate to provide patient-centered care
  • Identify high-risk patients and implement interventions to improve quality and safety
  • Make appropriate evidence-based, patient-centered medication recommendations
  • Participate in the management of medical emergencies
  • Provide discharge medication review, reconciliation, and counseling
  • Provide pharmaceutical services throughout the medical center
  • Complete critical patient monitoring and medication therapy reviews
  • Communicate effectively with healthcare providers and assure continuity of pharmaceutical care
  • Actively participate in stewardship activities and restricted medication programs
  • Participate in pharmacy operations and medication dispensing
  • Oversee pharmacy operations and technician practice/activities
  • Facilitate specialty medication procurement, ordering, and dispensing
  • Maintain proficiency and actively engage in education/mentorship
  • Develop staff training experiences/competencies and create new teaching opportunities
  • Identify, design, and implement improvements in the medication-use system
  • Graduate of an ACPE accredited College of Pharmacy
  • Clinical pharmacy experience (2-4 years) preferred (required if no PGY-2 or PGY-1 residency)
  • Knowledge of contemporary hospital/clinical practice and service
  • Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine
  • Able to communicate effectively with patients
  • Detail oriented
  • Doctor of Pharmacy or other advanced degree, preferred
  • Completion of ASHP accredited PGY-1 Pharmacy Residency, preferred
  • Completion of PGY-2 Specialty Pharmacy Residency, preferred
  • Leadership experience preferred
