The Clinical Pharmacist provides Pharmaceutical Care according to professional and departmental standards and statutory requirements, focusing on safe, effective, rational, and cost-conscious pharmacotherapeutic recommendations through medication management programs. This role involves assisting in supervising Pharmacy Technicians and Interns, participating in department-directed medication management activities, and demonstrating good judgment and decisiveness in daily activities. The Clinical Pharmacist is expected to maintain a working knowledge of current pharmaceutical practice and a personal commitment to professional growth. They will support management by providing oversight, guidance, and support for assigned areas, and participate in the planning, organization, implementation, and monitoring of new programs. Key responsibilities include maintaining expertise in drug therapy for patients of all age groups, communicating with other healthcare professionals to provide drug information and recommendations, and delivering clinical and operational pharmacy services. The role also involves documenting clinical activities, ensuring accurate drug therapy by checking for interactions, assisting with medication preparation and distribution, and providing patient education. Pharmacists will proactively assess prescribing patterns to promote safe medication use, identify cost avoidance strategies, participate in pharmacy decision-making, and engage in quality and safety programs. They are also expected to participate in educational and community programs, promote interdisciplinary communication, and serve as a role model for students, residents, and new hires. For outpatient pharmacies, administering vaccinations adhering to protocols and policies is also a duty.
