Under limited supervision, the Clinical Pharmacist provides clinical pharmacy services including primary care services, consultative services, and drug information services to patients and health care providers to deliver high quality care. This role involves reviewing prescriptions as written by authorized prescribers to determine that the order is safe and appropriate for the specific patient, contacting prescribers as necessary, and filling prescriptions by dispensing or compounding medication according to established procedures. The Clinical Pharmacist interviews patients for previous medication history, provides counseling on current drug therapy, and identifies patients requiring teaching, offering educational material. They also work collaboratively with other health care providers to assure coordination of patient education. A key aspect of the role is to assign, educate, train, and supervise support personnel (technicians, residents, interns, clerks, and students) in medication preparation and related duties, ensuring accuracy and compliance with policies. The position also involves ensuring maintenance of medication and pharmacy supplies inventory and related records, and compliance with environmental and regulatory agency procedures for storage. Responsibilities extend to providing formal patient and staff educational programs and conducting patient care area inspections. Clinical pharmacy services are provided in various specialized areas, including therapeutic drug monitoring, anti-coagulation service, nutritional support, investigational drug services, hematology/oncology pharmacy, medication use evaluation, pain management service, and adverse drug reaction reporting. Adherence to system, hospital, and departmental policies, procedures, and objectives is required.
