This position provides the services of a clinical pharmacist to the Blackburn Center clinic. The clinical pharmacist will serve as a link between CCC pharmacy and Clinic providers. The clinical pharmacist must be able to work independently, set priorities, make critical decisions, and respond to client concerns. The clinical pharmacist will serve the patient by preparing medications, giving pharmacological information to the health care team, and monitoring patient drug therapies. Clinical pharmacists will work directly with physicians, other health professionals, and patients to ensure that the medications prescribed for patients contribute to the best possible health outcomes. Clinical pharmacists practice in health care settings where they have frequent and regular interactions with providers and other health professionals, contributing to better coordination of care. The clinical pharmacist will also be involved in the education and training of pharmacy students. Blackburn Center Clinic (a designated Health Care for the Homeless program) provides primary care to a population of low-income adults with a broad range of physical, behavioral, and social conditions. Schedule: 3 days a week, to be determined , between Monday - Friday. Shift time likely 8:30am - 5:30pm or 9:00am - 6:00pm Location: Blackburn Center (12121 E. Burnside St. Portland, OR, 97216 Compensation: $142,000 - $184,000 annually. Range listed is for a 1.0 FTE, this role is 0.6 FTE so offer amounts would be based on that FTE amount.
