About The Position

This position provides the services of a clinical pharmacist to the Blackburn Center clinic. The clinical pharmacist will serve as a link between CCC pharmacy and Clinic providers. The clinical pharmacist must be able to work independently, set priorities, make critical decisions, and respond to client concerns. The clinical pharmacist will serve the patient by preparing medications, giving pharmacological information to the health care team, and monitoring patient drug therapies. Clinical pharmacists will work directly with physicians, other health professionals, and patients to ensure that the medications prescribed for patients contribute to the best possible health outcomes. Clinical pharmacists practice in health care settings where they have frequent and regular interactions with providers and other health professionals, contributing to better coordination of care. The clinical pharmacist will also be involved in the education and training of pharmacy students. Blackburn Center Clinic (a designated Health Care for the Homeless program) provides primary care to a population of low-income adults with a broad range of physical, behavioral, and social conditions. Schedule: 3 days a week, to be determined , between Monday - Friday. Shift time likely 8:30am - 5:30pm or 9:00am - 6:00pm Location: Blackburn Center (12121 E. Burnside St. Portland, OR, 97216 Compensation: $142,000 - $184,000 annually. Range listed is for a 1.0 FTE, this role is 0.6 FTE so offer amounts would be based on that FTE amount.

Requirements

• BS in Pharmacy from an accredited pharmacy school required. Doctor of Pharmacy preferred.
• Must be eligible for licensure in the State of Oregon, and obtained, prior to employment start date.
• Current CPR certification required prior to start.
• Available to work during standard clinic hours.
• Must meet CCC privileging requirement as required by FTCA.
• Ability to adhere to Central City Concern’s drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Workplace Ace of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess, or use a controlled substance or alcohol.
• Must pass a pre-employment drug screen, TB test, and DHS background check.
• If using a vehicle for agency use, must possess a current driver’s license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern’s Fleet Safety policy.
• Must pass an initial driver’s training within 60 days of being an approved driver and continued recertification training.
• Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.
• Physical ability to bend, stoop, kneel, squat, twist, reach, pull, and lift heavy objects.

Nice To Haves

• Doctor of Pharmacy preferred.
• PGY-1 residency training, (or equivalent training or experience) is preferred.
• Contemporary ambulatory pharmacy medication management and retail pharmacy operation management skills preferred.
• Bi-lingual, Spanish-speaking preferred.

Responsibilities

• Participate in the development of collaborative care protocols for several disease states, including but not limited to anticoagulation, asthma, COPD, diabetes, HepC, hypertension, hyperlipidemia, asthma, PrEP, and smoking cessation.
• Participate in the development and management of a Medication Therapy Management program.
• Participate in the development of a Pharmacy and Therapeutics Committee and perform drug usage and evaluation reports as needed for this committee.
• Ensure safe, appropriate, cost-effective medication therapies for patients according to the established laws, rules, policies, procedures, and protocols.
• Review medication orders for appropriateness and accuracy
• Prepare and dispense medication orders per prescriber requirements according to established policies, procedures, and protocols.
• Supervise and train pharmacy interns and externs.
• May serve as a preceptor as outlined by the State Board of Pharmacy.
• Provide in-service training to pharmacy and clinic staff as appropriate.
• Provide drug information service to providers and other healthcare professionals when requested or appropriate (e.g. cost-effective drug alternatives)
• Report adverse drug reactions, drug defects, and medication errors according to procedure.
• Collaborate and coordinate with care teams on patient care related to care protocols and medication therapy management.
• Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.
• Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
• Attend all mandatory CCC trainings in a timely manner.
• Perform other duties as assigned.
