We are currently conducting a search for a full-time Clinical Pharmacist to join a growing team of colleagues at our customer site, Mohawk Valley Health System in Utica, NY, to provide outstanding service to patients with complex, specialty diseases to help them achieve improved clinical outcomes. Pharmacy Location: Located within Mohawk Valley Health System in Utica, NY Schedule: Monday through Friday 8:00am - 5:00pm, 40 hours per week You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.

Requirements

  • PharmD or BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy accredited by American Association of Colleges of Pharmacy (AACP)
  • Pharmacist license in good standing in the state of New York
  • 1+ years of experience as a licensed pharmacist

Nice To Haves

  • Pharmacy residency or Board Certification
  • 1+ years of clinical experience or residency equivalent is preferred, but all candidates will be considered
  • Ambulatory or specialty experience
  • Proficiency in multiple EMR systems such as Cerner, EPIC or Meditech
  • Proficiency with dispensing systems such as PioneerRx, Willow Ambulatory, QS1, RX30, Computer Rx, etc. along with experience in pharmacy dispensing
  • Proficient in Microsoft Office (Word, Excel, PowerPoint, Outlook)

Responsibilities

  • Build solid working relationships with clinic staff and physicians while representing and supporting specialty pharmacy services
  • Lead medication management in assigned clinics, ensuring patients who may benefit are identified, engaged, and supported through in‑clinic interactions and follow-up protocols
  • Conduct clinical follow-ups via phone or in person, address inbound patient calls (including shared on-call coverage), and resolve adherence or therapy‑related issues promptly
  • Collaborate with physicians to ensure evidence‑based therapy decisions and participate in clinic/leadership meetings to support understanding of the CPS model
  • Assist with recruiting and training on-site Pharmacy Liaisons and Clinical Pharmacists, and support dispensing, administrative, and compliance tasks as needed
