This position is full-time (40 hours weekly, 8-hour shifts) that primarily works day shift with a midnight shift component and a weekend/holiday requirement; currently every 4th weekend and 1-2 holiday shifts per year. At the Union Campus, operations run through a network of inpatient pharmacists, including a central pharmacy and decentralized pharmacy coverage that focuses on critical care, emergency medicine, heart failure, neonates, oncology, pediatrics, perioperative services, sterile and non-sterile compounding. Clinical services include robust acute care services. In the acute care setting, the department employs a decentralized model of practice, placing pharmacists directly into patient care units as part of a multidisciplinary team. Requirements

  • Bachelor of Science degree in Pharmacy or PharmD degree from an accredited College of Pharmacy required
  • ASHP-accredited PGY-1 Pharmacy Practice Residency required; equivalent experience will be considered (3 years of acute care hospital experience)
  • Board certification preferred
  • Current licensure (or eligible to reciprocate) in the State of Maryland

Responsibilities

  • Participate in direct patient care through daily interdisciplinary rounds.
  • Manage specific medication therapies per approved policies (e.g., warfarin, aminoglycosides, vancomycin, IV to PO conversions, renal dose adjustments, indication-directed dosing adjustments, selection of surgical prophylaxis medications)
  • Evaluate drug efficacy, monitor therapeutic goals and outcomes, and review for contraindications/precautions
  • Order and evaluate laboratory tests per approved protocols to support medication therapy monitoring
  • Identify opportunities during chart review for counseling and medication history completion
  • Communicate recommendations directly with provider
  • Electronically document interventions and provide sign out as appropriate
  • Opportunities to serve as a preceptor for both pharmacy students and residents
  • Provide drug information to allied healthcare professionals and fulfill provider-initiated pharmacy consults
  • Assist with projects such as MUEs, drug monographs, and performance improvement activities
  • Review and interpret provider orders and dispense medication
  • Coordinate and supervise work-place activities
