About The Position

A key member of the pharmacy department team, this position requires a wide array of interpersonal as well as organizational skills. This person is responsible for accurate management of dispensing and clinical functions within the department, supervising technician and support personnel, provision of information to other professionals, computer order entry, and provision of appropriate patient care as a member of the pharmaceutical care team. Job responsibilities and assignments may vary with demonstrated skills and qualifications.

Requirements

  • Doctor of Pharmacy
  • One year of related experience. Requires a working knowledge of standard practices and procedures.
  • Georgia Pharmacist Licensure
  • Ability to interact and work well in a team-oriented environment.
  • Ability to work well in high-stress situations.
  • Needs computer skills in various program requirements.
  • Professional as well as pleasant demeanor when dealing with departmental, interdepartmental, and public personnel.

Responsibilities

  • Completing pharmacokinetics and other medication dosing consults and follow-up per THS protocol in an accurate and timely manner. Completing TPN consults and follow-up per THS protocol in an accurate and timely manner.
  • Ensuring safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols. Monitoring of drug therapy regimens for contraindications, drug-drug and drug-food interactions, allergies, and appropriateness of drug and dose. Reading, extracting, and interpretation of information from patient charts.
  • Interpretation of medication orders (verbal and written) and transcription to computerized patient medication profiles accurately and in a timely manner. Maintenance of accurate, complete patient medication profiles.
  • Preparation and dispensing of medication orders per physician request according to established policies, procedures, and protocols in an accurate and timely manner.
  • Verification of the daily activities and medication-related duties of the pharmacy technician staff.
  • Ability to adapt positively to frequent interruptions and changes in workload and/or work schedule.
  • Ability to organize and prioritize work assignments.
  • Ability to work independently with minimal supervision.
  • Appropriate and timely answering of the telephone and requests for the pharmacy staff.
  • Attendance at Pharmacy Department staff meetings.
  • Clinical consultation provision to healthcare practitioners, accurately and in a timely manner.
  • Compounding and dispensing of pharmaceuticals; including sterile products and parenteral nutrition products accurately.
  • Demonstration of sound professional judgment consistent with clinical/academic background.
  • Detection and reporting of suspected adverse drug events.
  • Documentation of clinical activities and interventions.
  • Formulary maintenance. Minimization of non-formulary procurements, utilization of therapeutic substitution protocols, and promotion of rational drug therapy selection.
  • Issuing controlled substances to patient care areas. Maintenance of accurate controlled substance records, as required by law.
  • Participation in the development and presentation of orientation, education, and training programs to the Pharmacy Department, medical, nursing, and other staffs, as assigned.
  • Participation in the quality improvement and medication use review activities of the department.
  • Provision of drug information to patients and families, accurately and in a timely manner.
  • Review and interpretation of culture and susceptibility data for antibiotic appropriateness. Recommendation of changes to therapy, as needed.
