About The Position

The pharmacist serves as part of the healthcare team by supporting authentic personalized relationships with patients and their chosen support system, collaborating with team members to assess, plan, implement and evaluate an individualized plan of care that promotes optimal health. The pharmacist accepts responsibility, authority, accountability and ownership for management and provision of care in accordance with current policies and procedures.

Requirements

  • Education: 4 Year/Bachelor's Degree required ndash; Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification.
  • Experience: 1 year of experience required - Postgraduate year 1 residency or fellowship OR a minimum of 2 years of experience as a registered pharmacist in health system pharmacy. Refer to the Life Support Training Policy for additional details.
  • Licensure: Current Pharmacist licensure in the state of Virginia completed within 30 days of start date by either examination or reciprocity. Board of Pharmacy Specialties Certification or other applicable certification must be obtained and continuously maintained within 2 years of start date as applicable to the role required; excludes PRN team members who may not be eligible to take certification exam. American Heart Association (AHA) Health Care Provider BLS certification required. American Heart Association (AHA) Health Care Provider ACLS certification - within six months of hire required.
  • Additional Skills/Requirements Required: Other duties as outlined in UVA Health Northern Virginia Culpeper and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.

Responsibilities

  • Provides optimal medication management and promotion of medication safety.
  • Performs patient assessments.
  • Participates in direct patient care activities and rounding opportunities where available.
  • Monitors therapy for appropriateness, dosage, effectiveness, interactions, and adverse drug reactions.
  • Reviews and acts upon clinical decision support surveillance.
  • Completes organizational and/or departmental required documentation.
  • Prescribes and/or administers medications as allowed by law and scope of practice.
  • Maintains up-to-date, evidence-based knowledge required for services provided.
  • Ensures timely and consistent communication.
  • Ensures timely responses to service line requests and inquiries.
  • Participates in patient education and wellness-based initiatives as driven by scope of practice.
  • Checks and controls quality and accuracy of work performed by pharmacy team members.
  • Prioritizes workflow ensuring all services are performed in a timely manner.
  • Maintains and follows formulary approvals, interchanges, stocked medications, and non-formulary process.
  • Monitors and enhances compliance with clinical programs.
  • Develops and delivers effective education to pharmacists and other healthcare providers.
  • Serves as a preceptor to residents and students.
  • Participates in the onboarding of pharmacists and other team members.
  • Provides ongoing feedback to fellow pharmacists to achieve desired outcomes.
  • Serves as a mentor to pharmacy team.
  • The incumbent may be asked to perform additional duties as assigned.
