About The Position

Position Summary: Reporting to our Assistant VP of Medical Affairs, the Clinical Pharmacist will join our team on a full time basis to provides medication management, medication adherence, medication reconciliation and medication education services. Our Clinical Pharmacist positively impacts our patient's ability to manage their chronic condition. This position is a hybrid role, requiring onsite duties based in the Bronx, NY along with some remote/work-from-home capabilities. Compensation- $120,000-$125,000 contingent on experience based on 40 hours per week. Schedule: Monday to Friday 8:00 am to 5:00 pm.

Requirements

• Doctor of Pharmacy Degree required from ACPE accredited college.
• NYS Licensed Pharmacist
• Must possess Postgraduate Year 1 (PGY1) and/or Postgraduate Year 2 (PGY2) in Ambulatory Care Residency

Nice To Haves

• Experience working with patients who suffer from Opioid Use Disorder, highly desired.
• Bilingual Spanish/English highly desired.

Responsibilities

• Takes Medical history of patient, medication reconciliation, and allergies.
• Provides patient education and counseling.
• Provides collaborative drug therapy management (CDTM) services.
• Maintains formulary, evaluates new formulary requests and maintains medication management policy and procedures.
• Provides tailor made therapeutic regimen, adjustment, and titration of doses for particular patients.
• Other clinical and administrative duties, as assigned.
