Join a team dedicated as a Clinical Pharmacy Specialist. This role is pivotal in providing safe, evidence-based, cost-effective, and patient-centered medication management as an integral part of the Malignant Hematology interdisciplinary team. You will support physicians, nurses, and pharmacists in both hospital and ambulatory clinic settings, optimizing patient outcomes through expert pharmaceutical care plans.

  • Review, clarify, and verify medication orders for accuracy, completeness, and appropriateness.
  • Develop, interpret, and deliver medication-related information and education to pharmacists, providers, nurses, and other healthcare professionals.
  • Coordinate and provide medication therapy education and counseling for patients and caregivers.
  • Lead the development, implementation, and maintenance of medication-related guidelines, policies, and standards.
  • Manage the medication-use process to enhance workflow efficiency, quality, and patient safety.
  • Ensure cost-effective medication utilization to optimize resources and minimize financial risk.
  • Doctor of Pharmacy Degree.
  • Board of Pharmacy Specialties (BPS) certification.
  • Extensive clinical experience within disease state specialization.
  • Experience in pharmacy practice, including medication safety, formulary management, pathway/guideline development, policy development/implementation, oncology reimbursement and coverage, and clinical research.
