About The Position

The Clinical Pharmacist will coordinate and help manage clinical programs and initiatives, including but not limited to, Medication Therapy Management (MTM), Annual Wellness Visits, Remote Patient Monitoring, Chronic Care Management, Adherence Programs, and Disease State Management. The role involves delivering MTM services, utilizing the Joint Commission of Pharmacy Practitioners (JCPP) Pharmacists’ Patient Care Process, and determining appropriate interventions to resolve medication therapy problems. The Clinical Pharmacist will also assist with managing medication costs, evaluate prescribed medication therapy, conduct patient-centered adherence services, and collaborate with prescribers and interdisciplinary care teams. The position requires maintaining knowledge of various quality measurement entities and meeting productivity and performance expectations.

Requirements

  • B.S. Degree or Pharm.D. in Pharmacy from an accredited educational institution.
  • Three years of experience as a practicing pharmacist or one year of experience plus completion of an Ambulatory Care Residency.
  • Current Minnesota, North Dakota, and Iowa pharmacist licenses.
  • Certificate of training for Immunizations.
  • Certification in CPR or BCLS.
  • Working knowledge of third party pay plans.
  • Excellent customer service and communication skills.
  • Proficient in the English language.
  • Attention to detail and ability to follow policies and procedures.
  • Ability to handle stressful situations and deal successfully with difficult customers.
  • Reliable attendance and punctuality.

Responsibilities

  • Deliver MTM services, including Comprehensive Medication Reviews (CMR) and Targeted Medication Reviews (TMR).
  • Utilize the JCPP Pharmacists’ Patient Care Process to deliver patient care.
  • Determine appropriate interventions to resolve medication therapy problems.
  • Use evidence-based practices to formulate recommendations and treatment plans.
  • Individualize patient care by evaluating indication, efficacy, safety, and adherence.
  • Assist with managing medication costs through cost-effective interventions.
  • Assess prescribed medication therapy for unnecessary medications and adherence concerns.
  • Evaluate and document clinical responses to therapy.
  • Conduct patient-centered adherence services.
  • Collaborate with prescribers and interdisciplinary care teams.
  • Provide patient-centered education on chronic disease states.
  • Contribute to a team culture promoting integrity, inclusivity, innovation, and accountability.
  • Maintain knowledge of quality measurement entities.
  • Meet productivity and performance expectations.
  • Perform all other duties assigned.
