Requirements

• Pharm.D. Degree.
• 2 years of experience in inpatient pharmacy hospital/clinical capacity or completion of Pharmacy Residency.
• Currently licensed or registered to practice pharmacy in Louisiana.

Nice To Haves

• Board Certified Pharmacotherapy Specialist (BCPS) preferred.

Responsibilities

• Direct and initiate development, implementation, and maintenance of clinical pharmacy services.
• Provide clinical consultation to the Medical Staff and other health care providers.
• Communicate and document all activities/interventions related to medication orders.
• Foster teamwork and take responsibility for high-quality patient care.
• Collaborate with health care providers to resolve concerns and questions.
• Provide ongoing education and drug information to healthcare professionals.
• Facilitate the development and implementation of treatment guidelines and protocols.
• Actively participate in clinical committees and act as a liaison to other healthcare practitioners.
• Demonstrate awareness of regulatory standards and departmental policies.
