As a Clinical Pharmacist in the Transplant department, you will play a vital role in ensuring patients receive their medications accurately, promptly, and with the utmost respect. You will use your expertise to select, dose, administer and dispense medications that support patient health and recovery, primarily focusing on heart transplant and heart failure care. In this role, you will round with heart teams, provide inpatient care, and see clinical patients as needed, while also being cross-trained to support liver and kidney transplant services. Requirements

  • PharmD or Bachelor's of Science in Pharmacy from an accredited College of Pharmacy.
  • Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire.
  • Current Basic Life Support (BLS) Certification through the American Heart Association (AHA).
  • Completion of a Board of Pharmacy approved course in the administration of immunizations or certification within 90 days of hire for Individuals administering immunizations.
  • Current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certifications through the American Heart Association (AHA) upon or within one of hire for individuals responding to adult or pediatric cardiopulmonary arrest events.

Nice To Haves

  • Board Certification.
  • Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience in Transplant or Critical Care.
  • Post-Graduate Year 2 Residency (PCY2) in Transplant or Critical Care.

Responsibilities

  • Provide safe, efficient, patient-centered and evidence-based pharmacy and medication services.
  • Ensure the appropriate selection, dosing, administration and dispensing of medication orders.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Document services in a complete, accurate and timely manner.
  • Actively contribute to medication safety initiatives.
  • Ensure all pharmacy operations are in compliance with national accreditation standards.
  • Educate and train students, residents, other caregivers, and/or patients.
  • Provide cost-effective care.
