About The Position

The Clinical Pharmacist Specialist is an advanced level position responsible for providing comprehensive clinical pharmacy services to patients of all ages at Phoenix Children's Hospital. This role involves medication therapy management, coordination of interdisciplinary programs, and serving as a resource for other departments. The specialist ensures the integration of pharmaceutical care programs and actively participates in the medication-use system, reporting directly to the Manager of Clinical Pharmacy Services.

Requirements

  • Advanced clinical pharmacy knowledge and experience in a hospital setting.
  • Current pharmacist license in Arizona or eligibility for licensure.
  • Strong communication and interpersonal skills to collaborate with healthcare teams.
  • Ability to manage complex medication therapy and provide patient education.
  • Experience with antimicrobial stewardship and medication reconciliation.

Nice To Haves

  • Board certification in a relevant specialty area (e.g., BCPS, BCIDP).
  • Experience in pediatric pharmacy practice.
  • Familiarity with electronic health record systems and medication management software.

Responsibilities

  • Maintain interdepartmental relationships to facilitate communication related to clinical pharmacy services and patient care.
  • Function as the advanced clinical expert and liaison for designated specialty services.
  • Coordinate and maintain the Antimicrobial Stewardship Program when serving as the Infectious Diseases Clinical Pharmacist Specialist.
  • Participate in patient and family education and support in transition of care to discharge.
  • Involved with medication reconciliation at the patient care level.
  • Develop guidelines, policies, and monitoring strategies for antimicrobial use.
  • Review and analyze daily antimicrobial audits for appropriate use in collaboration with the Infectious Disease Service.
  • Compile and report data on antimicrobial use to relevant committees.
  • Review and manage medication orders for therapeutic appropriateness based on patient information.
  • Screen for and manage clinical alerts/conditions related to medication orders.
  • Problem solve issues with medication orders and recommend alternative therapies as needed.
  • Comply with directives related to Formulary Management.
  • Document clinical interventions to meet cost containment and medication therapy goals.
  • Review, verify, or enter medication orders accurately and timely.
  • Communicate problems and unique patient needs to enhance continuity of care.
  • Ensure patient safety and clinical quality while providing pharmaceutical care.
  • Participate in patient safety committees and quality improvement initiatives.
  • Engage in adverse drug reaction reporting and medication error prevention initiatives.
  • Assist with managing therapies to support evidence-based practice.
  • Document pharmacist interventions and report results.
  • Lead and participate in managing drug shortages and recalls.
  • Attend emergency responses as required.
  • Assess, develop, and provide educational needs to pharmacy staff.
  • Provide educational lectures and in-services to pharmacy personnel and medical staff.
  • Provide advanced clinical preceptorship and training for pharmacy students and residents.
  • Perform drug usage evaluation reviews to promote rational drug therapy.
  • Prepare monographs and present at Pharmacy and Therapeutics Committee.
  • Participate in rounding with interdisciplinary teams to meet medication-related needs.
  • Engage in professional development programs to stay current in clinical practices.
  • Co-Chair and lead relevant committees as necessary.
  • Attend national meetings to enhance practice and publish information in pharmacy literature.
  • Maintain and author formulary, patient care, and medication safety policies.
