About The Position

The Clinical Pharmacist - Solid Organ Transplant at UW Medical Center - Montlake is responsible for providing comprehensive pharmaceutical care for solid organ transplant services. This role involves collaborating with a multidisciplinary patient care team, offering drug therapy consultations, developing treatment protocols, and educating patients, families, and staff. The position requires working in both inpatient and ambulatory clinic settings, ensuring safe medication management and transitions of care.

Requirements

  • Two years of experience in pharmacy practice.
  • Completion of an American Society of Health System Pharmacists accredited residency training program may substitute for experience.
  • State of Washington Pharmacist License.

  • PGY2 residency in solid organ transplant.

Responsibilities

  • Provide pharmaceutical care for solid organ transplant services.
  • Be a vital part of the multidisciplinary patient care team.
  • Provide drug therapy consults and participate in treatment protocol development.
  • Provide patient, family, and staff education.
  • Facilitate transitions of care and precept pharmacy students and residents.
  • Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.
  • Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.
  • Utilize medical records and documentation systems to communicate with healthcare professionals and document pharmacotherapeutic recommendations.
  • Provide drug information and education to medical teams, nursing, and dietitian staff.
  • Provide patient/family counseling and education, updating medication lists and reconciling orders upon admission and discharge.
  • Perform comprehensive medication therapy reviews to ensure safe transitions of care.
  • Monitor and modify patient medication therapy in collaboration with the medical team to achieve desired therapeutic outcomes in the ambulatory setting.
  • Provide primary preceptorship of pharmacy residents and student pharmacists.
  • Perform the duties of a Pharmacist 2.
